Forms and Information

Physical Form (.pdf, 177K)
Needed in order to participate in Collegiate Athletics.  Must be signed by Physician.  There is a fax machine available in the front athletics office if needed.

Insurance Form (.pdf, 183K)
Needed in order to participate in Collegiate Athletics.  Must be signed by Insurance Policy Holder.  If that is one of your parents, there is a fax machine available in the front athletics office.

Agreement to Participate (.pdf, 251K)
Needed in order to participate in Collegiate Athletics.  Must be signed by athlete or parent/guardian if under the age of 18.
